Subjetc: Access petition to personal data processed by your company.
Dear Sir/Madam
D. Arturo Gabriel Quirantes Sierra, over 18, living in xxxxx, with
DNI (ID Document) # xxxxx whose photocopy I'm encolsing, by the current
letter states his wish to enforce his right to access, pursuant to article 15,
Organic Law 15/1999, and articles 12,13, Royal Decree 1332/94.
PREVIOUS
The signer (me) travelled to the United States via your airline, on flight
IB6251, with departure date 26 March 2.003, leaving from Madrid-Barajas airport,
and arriving to John F. Kennedy Airport in New York, United States, I´d like to
know if those personal data, in your possession, have been transferred to United
States law-enforcement agencies, including, but not limited to, US Transport
Security Administration and US Customs and Border Protection of the United
States; if so, I´d like to know since when has this transfer been taking place.
The signer (me) writes and sends this request with the aim of
- Confirming whether my personal data have been processed by your company:
- Knowing which of my personal data is your company currently processing.
- Knowing the golas of this processing, kinds of data involved, and recipents -or
kinds or recipients- to receive those data.
- Obtaining those personal data in a readable form, as well as any other piece
of information related to which the source is.
- Knowing whether those personal data have been transferred outside [Spanish]
national territory, on any format and for any reason.
- Knowing the name of the person responsible for personal data processing in
your company.
With that goal, the signer
REQUESTS
1.- That free access is given to his files in, at most, one month from the
reception of this request, on the understanding that should that time pass
without an explicit answer to this petition, it will be considered as denied.
In that case, the proper protest will be made to the Data Protection Agency in
order to start a defense procedure, according to article 18, Organic Law and 17,
Royal Decree, not to mention any legal action that may be started.
2.- That, should the right of access petition be granted, it is sent by mail to
the above-written address in, at most, ten days after the right of access is
granted.
3.- That the information includes, in a legible way, the data concerning myself
that are included in your files, and the results of any processing or post-processing,
as well as the origin of the data, who yields them and for what uses have been
stored.
DNI (ID Card) photocopy is included.
